Reintervention after transcatheter edge to edge repair using MitraClip is still challenging. We aimed to report our experience in reinterventions after MitraClip procedures and describe the outcomes.
From 2012 to 2020, 167 patients had a transcatheter edge to edge repair; 10 of them needed reinterventions. At the time of the first MitraClip, the median EuroSCORE was 4.29 (2.62-7.52), and the ejection fraction was 30 (20-40)%.
Emergency mitral valve replacement (MVR) was performed in two patients, elective MVR in three, cardiac transplantation in two, and repeat clipping in threepatients. The median time from MitraClip to the reintervention was 4.5 (2-13) months. One patient required extracorporeal membrane oxygenation support after elective MVR. Repeat clipping failed to control mitral regurgitation grade in all patients. Clip detachment was reported in five patients (50%). The median follow-up after the reintervention was 19.5 (9-75) months, and mortality occurred in two patients who had repeat clipping (20%).

In this work, protective effect of A. kansuensis ethanol extract (AE) on pulmonary fibrosis was evaluated through paraquat (PQ)-induced pulmonary fibrosis animal model. Results showed that AE could significantly improve the survival rate, increase the body weight and reduce the lung index of mice at the raw drug doses of 700 and 350 mg/kg. Histopathological observation results showed that the destruction degree of lung tissue structure in mice was significantly improved with the increase of AE dosage. Collagen deposition in lung interstitium was significantly reduced. The marker protein alpha-SMA involved in PF were significantly inhibited through repressing TGF-beta1/Smads pathway. The degree of inflammatory infiltration was significantly reduced and inflammatory cytokines were significantly inhibited in mice through inhibiting the NF-kB-p65. Besides, oxidant stress level including upregulated ROS and down-regulated SOD and GSH was efficiently improved by AE through upregulation of Nrf2 and downregulation of NOX4. In summary, this study firstly showed that the protective effect of AE on pulmonary fibrosis was partly due to activation of Nrf2 pathway and the inhibition of NF-kB/TGF-beta1/Smad2/3 pathway.Public health researchers may have to decide whether to perform a meta-analysis including only high-quality randomized clinical trials (RCTs) or whether to include a mixture of all the available evidence, namely RCTs of varying quality and observational studies (OS). The main hurdle when combining disparate evidence in a meta-analysis is that we are not only combining results of interest but we are also combining multiple biases. Therefore, commonly applied meta-analysis methods may lead to misleading conclusions. In this paper, we present a new Bayesian hierarchical model, called the bias-corrected (BC) meta-analysis model, to combine different study types in meta-analysis. This model is based on a mixture of two random effects distributions, where the first component corresponds to the model of interest and the second component to the hidden bias structure. In this way, the resulting model of interest is adjusted by the internal validity bias of the studies included in a systematic review. We illustrate the BC model with two meta-analyses The first one combines RCTs and OS to assess effectiveness of vaccination to prevent invasive pneumococcal disease. Our results show that ignoring internal validity bias in a meta-analysis may lead to misleading conclusions. The BC model has been implemented in JAGS and R, which facilitate its application in practice.The present study is aimed to evaluate the effects of different levels of earthworm (EW) powder and vermihumus (VH) on the growth performance, humoral immunity response, plasma constituents and reproductive performance of Japanese breeder quails. A total of 336 birds were assigned to seven treatments in a completely randomized design from day 30 to 90 of age. The dietary treatments were carried out, one without any addition of EW and VH, the second had a standard rate of VH at 0.8%, while a range of EW additions was supplied from 0%, 0.5%, 1%, 1.5%, 2% and 2.5%. The effects of treatments on body weight and mortality were not significant (p > .05). The highest feed intake was observed in birds fed the diets supplemented with 1% and 1.5% EW (p less then .05). The highest titre of antibodies against influenza virus and sheep red blood cells belonged to the EW and VH treatments (p less then .05). The antibody titre against Newcastle disease virus vaccine was not affected by the treatments. In general, the birds receiving EW and VH had the highest egg weight, egg production and egg mass weight (p less then .05). Nonetheless, various levels of EW powder negatively affected hatchability and increased mortality and leg disorders (p less then .05). The results showed that the levels of dietary 1% and 1.5% of EW powder gave the best growth and reproductive performance to birds respectively.
